Apply for Membership

To join NQI you must complete a Membership Application Form. To download the PDF form, click here.

Send the completed form to NQI by fax, (416) 251-9131, or by email to membership@nqi.ca. If you would prefer to receive a Microsoft Word document which you can fill in and email back, let us know and we’ll email you the form. Online application coming soon!

Upon receipt of the completed Membership Application form you will be contacted by the Relationship Manager who will put your benefits in place as quickly as possible and keep you posted regarding new benefits as they are added.
